We track CMS enforcement actions, inspection records, staffing data, and quality measures across multiple states. We identify patterns that individual families often cannot see on their own.
Medical documentation, audit trails, MDS assessments, and regulatory deficiency reports contain the evidence of neglect. We explain what that language means so families can use it.
Awareness without tools is just noise. Every resource on this site is designed to help a family take the next step: document concerns, file a complaint, request records, or find legal help.
How We Started
Built From the Inside of the System
Silent Voices Elder Advocacy was founded in 2026 by Nathalie Frias, a Certified Electronic Health Records Specialist who spent years reviewing medical records in the context of malpractice litigation. In that work, a pattern emerged: the documentation that was supposed to protect residents was being used to conceal the failures that harmed them.
Ghost charting. Cloned assessments. Altered audit trails. Schizophrenia diagnoses added retroactively to suppress antipsychotic counts. These were not isolated errors. They were patterns, and they appeared across facilities, across states, and across ownership structures.
SVEA was built to close the gap between what the records say and what families understand. The long-term care system produces enormous amounts of documentation. Very little of it is written for the people who need it most. We change that.
Silent Voices Elder Advocacy operates as an independent platform. We are not a journalism outlet, and we do not conduct investigations in a law enforcement capacity. We research publicly available data, translate clinical and regulatory language, and connect families to the tools and resources they need to take action.
The Founder
Nathalie Frias is a Certified Electronic Health Records Specialist and Certified Medical Administrative Assistant with professional experience in medical records review, health information management, and long-term care documentation. Her background in reviewing clinical records for malpractice litigation gave her direct exposure to the ways documentation practices in nursing facilities conceal neglect, alter timelines, and undermine accountability.
She is also a published psychological thriller author whose fiction draws directly from the patterns she has observed in institutional care settings. Her writing translates clinical documentation into evidence families and advocates can use to expose neglect, financial exploitation, and regulatory failures in long-term care.
Nathalie founded Silent Voices Elder Advocacy in 2026 to build the resource she wished had existed for the families whose records she reviewed over the years. She holds a Board Certified Patient Advocate exam window for October through November 2026 and is based in Arkansas.
